Background Inadequate knowledge among health care providers (HCPs) and parents of affected children limits the understanding and utility of secondary genetic findings (SFs) in under-represented populations in genomics research. SFs arise from deep DNA sequencing done for research or diagnostic purposes and may burden patients and their families despite their potential health importance. This study aims to evaluate the perspective of both groups regarding SFs and their choices in the return of results from genetic testing in the context of orofacial (...) clefts.Methods Using an online survey, we evaluated the experiences of 252 HCPs and 197 parents across participating cleft clinics in Ghana and Nigeria toward the return of SFs across several domains.Results Only 1.6% of the HCPs felt they had an expert understanding of when and how to incorporate genomic medicine into practice, while 50.0% agreed that all SFs should be returned to patients. About 95.4% of parents were willing to receive all the information from genetic testing (including SFs), while the majority cited physicians as their primary information source (64%).Conclusions Overall, parents and providers were aware that genetic testing could help in the clinical management of diseases. However, they cited a lack of knowledge about genomic medicine, uncertain clinical utility, and lack of available learning resources as barriers. The knowledge gained from this study will assist with developing guidelines and policies to guide providers on the return of SFs in sub-Saharan Africa and across the continent. (shrink)

Until now, adult crying has received relatively little interest from investigators, whereas in the popular media there are many strong claims about crying (e.g., crying brings relief) of which the scientific basis is not clear. In this review, we provide an overview of the current state of the scientific literature with respect to crying. We identify gaps in knowledge and propose questions for future research. The following topics receive special attention: Ontogenetic development, antecedents, individual and gender differences, and the intra- (...) and interindividual effects of crying. We conclude that the study of crying may help us obtain better insight into human nature, that is, not only our emotional, but also social, and moral functioning. (shrink)

In this volume, the sixth in Blackwell's Great Debates in Philosophy series, Smart and Haldane discuss the case for and against religious belief. The debate is unusual in beginning with the negative side. After a short jointly authored introduction, there is a fairly extended presentation of the atheist position by Smart. Haldane then offers an equally extended defense of theism. The authors respond to one another in the same order, and the book concludes with a brief co-authored treatment of antirealism, (...) which both reject. The discussion is broad-ranging, in part reflecting the size of the topic. It addresses the existence of God and the problem of evil, along with related issues having to do with God's nature and his involvement with the world: necessary being, eternity versus sempiternity, the relation between divine sovereignty and human freedom, and the possibility of miracles. But there is also discussion of subjects having to do with revealed religion, such as the reliability of scripture, and the roles of faith and reason in the life of the believer. The fact that the book commences with the case for atheism also contributes to its scope, since without a specific version of theism on which to focus, Smart has to aim broadly. The result is that topics that might otherwise have been ignored—moral arguments, and Pascal's wager, to cite two—are at least broached, though not pursued. (shrink)

Respect for autonomy is well known as a core element of normative views on good care. Most often it is interpreted in a liberal way, with a focus on independence and self-determination. In this article we argue that this interpretation is too narrow in the context of care in nursing homes. With the aim of developing an alternative view on respect for autonomy in this setting we described four interpretations and investigated the moral intuitions (i.e. moral judgements) of caregivers regarding (...) these approaches. We found that these caregivers seemed to value different notions relating to respect for autonomy under different circumstances. There was no significant difference in moral judgements between men and women or between doctors and nurses. We conclude that a multidimensional understanding of this principle would best fit this context. We end this article with a description of a modest theory of respect for autonomy in nursing homes. Multiple accounts of how theories of human psychology and of image-making influenced each other in a decisive period in the history of philosophy and art.

This paper addresses a worry about backwards time travel. The worry is that there is something mysteriously inexplicable about the combination of commonplace events that will inevitably conspire to prevent the time traveler from doing something impossible such as killing her younger self. The worry is first distinguished from other problems for backwards time travel concerning its alleged impossibility or improbability. It is then shown that the worry is misplaced: there is in fact no real problem here. Yet the worry (...) has been widely expressed—so a suggestion is also made as to why it is so easy to get into the position of thinking that there is a genuine problem here, when in fact there is not. Finally, in light of the resolution of the inexplicability worry, a new way of dealing with the other two problems for backwards time travel—concerning its alleged impossibility and improbability—is proposed. In this paper I present a new theory of propositions, according to which propositions are abstract mathematical objects: well-formed formulas together with models. I distinguish the theory from a number of existing views and explain some of its advantages  chief amongst which are the following. On this view, propositions are unified and intrinsically truth-bearing. They are mind- and language-independent and they are governed by logic. The theory of propositions is ontologically innocent. It makes room for an appropriate interface with (...) formal semantics and it does not enforce an overly fine or overly coarse level of granularity. (shrink)

One of Tarski’s stated aims was to give an explication of the classical conception of truth—truth as ‘saying it how it is’. Many subsequent commentators have felt that he achieved this aim. Tarski’s core idea of defining truth via satisfaction has now found its way into standard logic textbooks. This paper looks at such textbook definitions of truth in a model for standard first-order languages and argues that they fail from the point of view of explication of the classical notion (...) of truth. The paper furthermore argues that a subtly different definition—also to be found in classic textbooks but much less prevalent than the kind of definition that proceeds via satisfaction—succeeds from this point of view. A requirement on any theory of vagueness is that it solve the sorites paradox. It is generally agreed that there are two aspects to such a solution: one task is to locate the error in the sorites argument; the second task is to explain why the sorites reasoning is a paradox rather than a simple mistake. I argue for a further constraint on approaches to the second task: they should conform to the standard modus operandi in formal semantics, in which (...) the semantic theory one develops is taken to be implicit in the ordinary usage of competent speakers. Thus it should not turn out that one's explanation of why ordinary speakers react to the sorites reasoning in the way they do depends on speakers not thinking that the semantics of vague language is governed by the theory one is advocating. I then argue that, out of the current main contenders for a theory of vagueness, only theories that posit degrees of truth can meet this further constraint. (shrink)

Clinical red blood cell transfusion guidelines have been widely adopted in clinical practice, resulting in standardized transfusion practices in hospitalized patients with anemia. Standardization of transfusion practice has been welcomed by clinicians and health systems as a mechanism for reducing unnecessary, harmful, and costly practice variation that results in healthcare disparities. However, overzealously applied guidelines can have deleterious consequences for individual patients, ultimately resulting in and/or exacerbating healthcare disparities, rather than resolving them. This article provides empirical examples of the adverse (...) consequences from the well-meaning attempt to standardize transfusion practice based on clinical practice guidelines and discusses the ethical implications of standardized transfusion practice. Cet article fournit les principaux résultats de la mission effectuée en mai 2011 dans le cadre du projet concernant la sculpture architecturale byzantine de la côte occidentale de la mer Noire. La première partie présente et décrit les ensembles architecturaux d’où proviennent les sculptures sur lesquelles ont été prélevés des échantillons de marbre. La seconde partie a trait aux caractéristiques spécifiques des marbres analysés : vingt échantillons de marbre prélevés sur des sculptures provenant surtout de Sozopol, mais aussi d’Obzor et (...) de Pomorie, ont été analysés afin de connaître leur provenance. Les différentes méthodes d’analyses ont montré que la majorité des échantillons étaient issus de l’île de Proconnèse. Néanmoins, quatre des vingt échantillons présentent une composition minéralogique et chimique différente de celle des marbres de Proconnèse : deux sont en marbre pentélique, un autre en marbre de Thasos et le dernier est très probablement en marbre local. This article indicates some ways in which the fantasy of the brain in the vat has been used in thought experiments to discuss important philosophical problems. The first has to do with scepticism about the external world. The second has to do with Hilary Putnam’s arguments for the indeterminacy of reference and his rejection of metaphysical realism. The third issue to which the brain in the vat is relevant has to do with the difference between broad and narrow content of (...) beliefs and Putnam’s challenging assertion that reference is not ‘in the head’. Though there are brief suggestions for dealing with these problems and saving metaphysical realism, the main purpose is to indicate the relevance of the fantasy of the brain in the vat to discussion of these issues. This volume comprises systematic as well as historical essays, including contributions intended to give comprehensive overviews of such areas as genetic phenomenology, transcendental phenomenology, philosophy and history of logic and mathematics, Kant, hermeneutics, Hegel, and philosophy of language. The book is addressed to phenomenologists, particularly those who are interested in some or all of the areas mentioned. In his introduction Joseph J. Kockelmans indicates that these diverse areas enter into dialogue in the work of Thomas M. This paper intends to show that it is possible to take convincingly into account the platitudes which underlie our moral practice. It deals with the articulation of an indirect strategy to answer the arguments of moral scepticism: if we can generate a conceptual space that takes such platitudes into account, perhaps we could ignore the sceptic doubts.Several metaethical conceptions can ecumenically carry out this task. Particularly, even if moral realism implies cognitivism and objectivism, moral objectivism –the plausibility of rational agreement (...) in morals- does presuppose necessarily neither realism nor cognitivism.An argument against moral relativism is also presented. It is argued that there are several ways of pluralism compatible with the very field of moral objectivity.The conclusion maintains that only an objectivist account of our moral practice is apt to make sense of our institutional designs that acknowledge universal human rights. The storage hypothesis—as described by Norby—is a descriptive thesis (for it yields systematic predictions of human behaviour across a wide range of situations) that has as a core commitment that degrees of belief are stable, persistent states. It is not clear to me that such a view is widely held in philosophy. If the storage hypothesis is not widely held, then arguments against it become less interesting. But is Norby’s argument against the view compelling in any case? I shall argue (...) that it is not. (shrink)

I come to praise contemporary French philosophy not to bury it. My aim is not to hail the appearance in France of a native brand of analytic philosophy—in itself an important event in the last decade—but to describe the indirect and selective importation of certain Anglo-Saxon concepts by French philosophers whose practice is far from analytic; and also to describe the resultant misunderstanding. In this paper I shall analyse the use of pragmatic concepts—and of the concept ‘pragmatics’—in the recent work (...) by Deleuze and Guattari, Mille Plateaux, and I shall try to show that these concepts are the object of a creative misunderstanding, of a misprision.
